  1. Metastatic CRC After Progression
Progression after at least 2 prior systemic therapy lines, including standard chemotherapy.
    • Reassess Clinical Status And Biomarkers
This algorithm is intended for patients with refractory metastatic colorectal cancer after appropriate previous systemic treatments. Review ECOG performance status, comorbidities, prior therapies, cumulative toxicities, molecular results (BRAF, HER2, KRAS, MSI-H, and dMMR), and patient preferences. Treatment eligibility should be determined by actual prior exposure rather than line number alone.
      • Eligible for Unused Biomarker-Directed Therapy?
YES: An appropriate therapy for which the patient satisfies molecular, prior-treatment, and clinical eligibility criteria and that has not already failed. NO: No currently eligible biomarker-directed therapy.
        • Biomarker-Directed Therapy
BRAF V600E: Encorafenib + cetuximab when appropriate and not previously used. KRAS G12C: Sotorasib + panitumumab or adagrasib + cetuximab after required prior chemotherapy. HER2-positive, RAS WT: Tucatinib + trastuzumab when eligible. HER2 IHC 3+: Trastuzumab deruxtecan when eligible. MSI-H/dMMR: Checkpoint immunotherapy if appropriate and not previously received. Rare actionable alterations: Appropriate tumor-agnostic therapies, including qualifying NTRK or RET fusions.
          • Monitor Treatment Response and Toxicity
Monitor treatment and consider clinical trials at any point.
        • Candidate For Further Systemic Therapy?
Assess fitness, anticipated benefit, toxicity, comorbidities, and patient goals.
          • Trifluridine/Tipiracil + Bevacizumab, Fruquintinib, or Regorafenib
Consider trifluridine/tipiracil plus bevacizumab, fruquintinib, or regorafenib based on prior therapy, toxicity, comorbidities, and preferences. These treatments are not always interchangeable. The June 2026 ESMO guideline recommends: Trifluridine/tipiracil + bevacizumab: Generally preferred when eligible and not previously used. Regorafenib: An option after trifluridine/tipiracil + bevacizumab. Fruquintinib: Generally considered after progression on trifluridine/tipiracil or regorafenib, with required prior chemotherapy and biologic exposure.
            • Monitor Treatment Response and Toxicity
Monitor treatment and consider clinical trials at any point.
          • Best Supportive Care
Provide palliative care, symptom management, and hospice when appropriate.
