  1. Metastatic CRC With Progression on or After First-Line Therapy
    • Reassess Prior Regimens, Tolerability, Performance Status, and Biomarker Results
Check molecular testing completeness, prior anti-VEGF and anti-EGFR exposure, previous immunotherapy, cumulative neuropathy, ECOG performance status, and the possibility of local treatment for selected oligoprogressive disease. Review MSI/MMR, RAS, KRAS/NRAS, BRAF V600E, HER2, KRAS G12C, and other results.
      • Eligible for Biomarker-Directed Therapy Now?
        • Select Biomarker-Directed Therapy After Required Prior Therapy
BRAF V600E: encorafenib plus cetuximab. HER2+: HER2-directed therapy. KRAS G12C: targeted combination. MSI-H/dMMR: immune checkpoint inhibitor.
          • Reassess Response, Tolerability, and Goals of Care
Consider a clinical trial and later-line options if progression occurs.
        • Which Chemotherapy Backbone Was Used in First Line?
          • Oxaliplatin-Based First-Line Therapy
            Examples include FOLFOX or CAPOX.
            • FOLFIRI + Anti-VEGF or Anti-EGFR Therapy if Eligible
Use FOLFIRI with an anti-VEGF agent or, when eligible, an anti-EGFR agent as the alternative biologic strategy; do not combine anti-VEGF and anti-EGFR therapy. Bevacizumab is a commonly preferred anti-VEGF option, with ziv-aflibercept or ramucirumab alternatives where appropriate. For eligible left-sided RAS-wild-type disease without prior anti-EGFR treatment, use cetuximab or panitumumab with an appropriate chemotherapy backbone instead of anti-VEGF therapy.
              • Reassess Response, Tolerability, and Goals of Care
Consider a clinical trial and later-line options if progression occurs.
          • Irinotecan-Based First-Line Therapy
            Example: FOLFIRI.
            • Clinically Significant Oxaliplatin Intolerance or Persistent Neuropathy?
              • FOLFOX or CAPOX ± Bevacizumab; Consider Anti-EGFR if Eligible
Use FOLFOX or CAPOX with or without bevacizumab; consider anti-EGFR therapy when appropriate.
                • Reassess Response, Tolerability, and Goals of Care
Consider a clinical trial and later-line options if progression occurs.
              • Individualize Next-Line Therapy Based on Prior Exposure and Biomarkers
                • Reassess Response, Tolerability, and Goals of Care
Consider a clinical trial and later-line options if progression occurs.
          • Individualize Next-Line Therapy Based on Prior Exposure and Biomarkers
            • Reassess Response, Tolerability, and Goals of Care
Consider a clinical trial and later-line options if progression occurs.
          • Select Initial Chemotherapy Backbone Based on Biomarkers and Fitness
