  1. Primary LG Ta
    • Flexible cystoscopy at 3 mo
      • Risk-adapted follow-up
        • Recurrence
          • High probability of LG
            • Active surveillance
              In case of ≤2 IBCG IR-NMIBC risk factors (especially if low tumor burden)
            • Office fulguration / laser ablation
            • TURBT
          • HG appearance or positive UC or diameter >2–3 cm
            • TURBT
        • Absence of recurrence
          • Discontinue cystoscopic surveillance after 5 yr
